Arbitration Forms
The Secretariat of the QSAT periodically issues documents for the information of parties and arbitrators, as well as to aid the conduct of the proceedings. In addition, the Forms listed below are QSAT official forms to be used as the case may be.
QSAT reserves the right to update these documents at any time.
Submission of Request for Arbitration
A Request for Arbitration may be submitted by post, courier or hand delivery. It may alternatively be submitted by email or facsimile, provided that the requisite number of hard copies follows by post, courier or hand delivery. Requests are accepted at the following address:
